This form applies only to your 2025 Assessment, and for 2024 Supplementary Assessments.
The deadline for submitting this form is 11:59 p.m. PST on January 31, 2025.

Note: Once this form is submitted you will receive a confirmation of your submission by email. If you do not receive a confirmation email, please check your junk mail folders or call your local Assessment Office to confirm it was received. Any supporting documentation (photos, etc.) for your appeal can be submitted by uploading to the PARP Online Evidence Submission System.
